Among the more than 90 identified genetic risk loci for late-onset Alzheimer's disease (AD) and related dementias, the apolipoprotein E (APOE) gene ɛ2/ɛ3/ɛ4 polymorphisms remain the longstanding benchmark for genetic disease risk with a consistently large effect across studies1-10. Despite this massive signal, the exact mechanisms by which ɛ4 increases and ɛ2 decreases dementia risk remain poorly understood. Notably, recent trials of anti-amyloid therapies suggest less efficacy and higher risks of severe side effects in ε4 carriers11-13, hampering the treatment of those with the highest unmet need. To improve our understanding of the genetic architecture of AD in the context of its main genetic driver, we performed genome-wide association studies (GWASs) stratified by ε4 and ε2 carrier status. HP1BP3, SLC50A1, PTPRC, NPAS3, DDHD1, CHST9, SMYD2, PRAMEF1 and GFRA1 emerged as new genomic signals for AD risk, appearing only when stratified by APOE carrier status. DDHD1 appeared especially promising, showing protective effects in ε4 carriers, being identified as an expression quantitative trait locus and being involved in rare neuronal diseases. Such APOE-stratified insights may help understand and overcome side effects, inform clinical trial enrollment strategies, and create the scientific basis for targeted, mechanism-driven therapies in neurodegenerative diseases.

Understanding the molecular basis of neurodegeneration requires a comprehensive map of the genome's protein-coding output. Although thousands of small open reading frames (ORFs) are translated in the human brain, proteomic evidence for their encoded microproteins (MPs) (≤150 amino acids (aa)) remains limited. Here, we present a brain MP atlas that integrates transcriptomics, mass spectrometry and deep-learning-predicted spectra across more than 600 postmortem frontal cortex samples with and without Alzheimer's disease (AD). We identified 1,067 MPs absent from reviewed UniProtKB entries with high-confidence spectral support. A subset is differentially expressed in AD independently of the annotated main ORF at the same locus. A small ORF expressed by MKKS encodes a 63-amino-acid MP that is the locus's predominant translation product and is downregulated in AD; its loss impairs microglial mitochondrial respiration, implicating it in microglial bioenergetics. This atlas expands the annotated brain proteome and provides a resource for studying MPs in aging and neurodegeneration.

Lipid droplets are dynamic cellular organelles that store neutral lipids and coordinate metabolic and stress-response pathways. In the brain, lipid droplets in glial cells, including astrocytes, have been implicated in Alzheimer's disease, but how genetic risk factors influence their composition and turnover remains poorly understood. APOE is the strongest genetic modulator of late-onset Alzheimer's disease and exists in common variants that confer decreased, neutral, or increased risk. Here we show that APOE genotype shapes the lipid droplet proteome, lipidome, and degradation dynamics in human induced pluripotent stem cell-derived astrocytes. By comparing oleic acid-treated astrocytes carrying APOE2, APOE3, or APOE4, we find that each variant is associated with distinct lipid droplet proteins and lipids. These molecular differences correspond to genotype-dependent changes in lipophagy, an autophagy-mediated pathway for lipid droplet clearance. Lipid droplets in APOE2 astrocytes undergo efficient autophagic turnover, whereas those in APOE4 astrocytes resist degradation. These findings identify impaired lipid droplet clearance as a potential mechanism linking APOE4 to Alzheimer's disease risk.

Tau aggregation is a central pathological feature of Alzheimer's disease, yet how different forms of tau-ranging from monomers to small soluble aggregates and mature fibrils-interact with the cellular environment remains poorly understood. Here, we combine immunoaffinity proteomics with single-molecule techniques and super-resolution microscopy to systematically map the tau interactome across defined aggregation states, spanning monomeric tau, nanoscopic soluble aggregates, and fibrillar species. Using post-mortem Alzheimer's disease brain tissue, we identify distinct functional modules associated with different aggregation states: while proteostasis factors and immune-related proteins preferentially associate with nanoscopic aggregates (oligomers), cytoskeletal, metabolic, and RNA-binding proteins are enriched for mature fibrillar tau. Single-molecule microscopy directly confirms this conformation-dependent recruitment for key interactors including Hsp70-2, ENO1, hnRNPA1, APP, EAAT4, and ubiquitin. A primary-neuron system with accelerated tau aggregation is used to model these findings in a controlled system, showing striking similarities to the brain samples. Finally, pseudotime analysis reconstructs a progressive remodelling of the tau interactome across disease progression, revealing stage-specific pathway vulnerabilities. Together, these results establish a temporally resolved framework for tau pathology shaped by protein interactions and identify potential therapeutic intervention points for investigation across stages of disease.

Alzheimer's disease (AD) involves complex neuroimmune dysregulation, and the role of immune checkpoint pathways in regulating neuro-glial interactions and intrinsic glial homeostasis remains unclear. In AD, glial expression of programmed cell death protein 1 (PD-1) and its ligand (PD-L1) is elevated in mouse models and human patients, suggesting involvement of immune checkpoint signaling in glial function. To define the role of this pathway in the AD brain, we locally modulated PD-1/PD-L1 signaling by intracortical anti-PD-L1 injection in 8-10-month-old 5xFAD mice, combined with in vivo two-photon imaging and quantitative functional analyses. Brain-intrinsic PD-L1 blockade reshaped the local glial microenvironment, restoring impaired microglial process convergence and increasing P2RY12 expression, a key marker of homeostatic function, with concomitant attenuation of aberrant neuronal hyperactivity. Astrocyte-specific PD-L1 knockdown produced similar effects, indicating a key role of astrocytic PD-L1 in regulating microglia-neuron interactions. These findings suggest that the glial PD-1/PD-L1 axis functions as a brain-intrinsic regulator of glial homeostasis linked to neuronal dysfunction in AD.

INTRODUCTION: Amyloid-beta (Aβ)-induced microvascular injury is a key pathological feature of cerebral amyloid angiopathy (CAA). Recent evidence suggests that ferroptosis is implicated in Aβ pathology; however, its cell-type specificity and regulatory mechanisms within the cerebral microvasculature in CAA remain unclear. METHODS: Single-cell RNA sequencing (scRNA-seq) was performed on cortical microvessels from 11-month-old APP23 mice and wild-type littermates to identify cell-type-specific ferroptosis signatures. Biochemical, histopathological, and intervention experiments were further conducted to validate the findings and investigate the underlying mechanisms. RESULTS: Venous endothelial cluster exhibited the most prominent enrichment of ferroptosis signatures in scRNA-seq analysis, with subsequent experiments demonstrating progressive ferroptosis-related alterations in cortical venular endothelial cells (vECs) during CAA development. Mechanistically, Aβ1-40 increased activating transcription factor 3 (ATF3) expression and its occupancy at the Slc7a11 promoter, whereas ATF3 silencing restored Slc7a11/xCT expression, supporting ATF3-mediated Slc7a11/xCT repression and consequent impairment of glutathione-dependent antioxidant defense. Importantly, endothelial ATF3 knockdown attenuated ferroptosis-related alterations in vECs, reduced cerebral Aβ burden, and ameliorated cognitive deficits in APP23 mice. DISCUSSION: This study uncovers a previously underrecognized Aβ-driven microvascular injury characterized by vEC ferroptosis and highlights the ATF3/xCT axis as a potential therapeutic target in the early stage of CAA.

INTRODUCTION: Lysosomal dysfunction contributes to Alzheimer's disease (AD) by impairing protein clearance and promoting neuroinflammation. Cathepsin H (CTSH), a lysosomal protease, recently emerged as a protective AD locus. We investigated how CTSH is regulated and how it influences early AD pathophysiology. METHODS: We analyzed genomic, transcriptomic, and proteomic data from cerebrospinal fluid (CSF) and brain tissue across three independent clinical and post mortem cohorts to assess CTSH regulation, expression, and disease associations. RESULTS: The coding variant rs2289702 acts as a cis-regulatory variant, altering CTSH mRNA and protein levels. The T allele associates with better cognition and reduced amyloid plaque burden. CSF CTSH correlates with total tau, phosphorylated tau181, neuronal markers, and multiple glial and complement-related inflammatory proteins. DISCUSSION: CTSH tracks early neurodegenerative, synaptic, and inflammatory changes, and co-expression analyses link it to broader immune-metabolic pathways. The findings position CTSH as a genetically regulated contributor to AD pathophysiology.

INTRODUCTION: Amyloid positivity does not reliably predict near-term cognitive decline in preclinical Alzheimer's disease (AD). We tested whether digital cognitive phenotyping improves prognostic precision. METHODS: Baseline performance on a brief self-administered digital battery was analyzed in 1146 amyloid beta (Aβ)+ and 538 Aβ- cognitively unimpaired older adults from the Anti-Amyloid Treatment in Asymptomatic Alzheimer's/Longitudinal Evaluation of Amyloid Risk and Neurodegeneration studies. Unsupervised k-means clustering identified cognitive phenotypes. Survival models examined time to Preclinical Alzheimer Cognitive Composite decline and Clinical Dementia Rating (CDR) progression. Associations with tau positron emission tomography and plasma phosphorylated tau (p-tau)217 were evaluated. RESULTS: Four cognitive phenotypes emerged. One phenotype (18% of Aβ+ participants) showed the steepest cognitive decline, highest progression on the CDR, greater baseline medial temporal tau, and fastest neocortical tau accumulation over 4.6 years (hazard ratio [HR] for decline ≈ 2.9). Combining this phenotype with elevated plasma p-tau217 identified a subgroup at markedly amplified risk (HR ≈ 4.5). DISCUSSION: Digital cognitive phenotyping, especially when integrated with plasma biomarkers, identifies actionable risk heterogeneity and enables scalable precision prognostication in preclinical AD.

Alzheimer's disease (AD), especially its moderate-to-severe stage (MSAD), remains a global health challenge with no disease-modifying therapies. Here, we identified a dual pathological signature in MSAD patients and murine models: a systemic pro-inflammatory milieu and critically reduced brain-derived neurotrophic factor (BDNF) levels in both the hippocampus and serum. To mitigate the decrease in BDNF, we developed an engineered neutrophil capsule, in which neutrophils carrying lipid nanoparticles encapsulating BDNF penetrated the brain and ameliorated cognitive capacity deficits in MSAD mice. The pro-inflammatory state in the MSAD brain facilitated neutrophil passage through the blood-brain barrier, and inflammatory mediators triggered the release of BDNF from the engineered neutrophils. Collectively, our findings show that engineered neutrophil-mediated brain delivery of BDNF efficiently reduces the amyloid-β plaque deposition in the hippocampus, leading to significantly improved learning and memory in MSAD mice. This study establishes neutrophil-mediated delivery as a paradigm strategy for MSAD, merging cell biology with nanotechnology to reverse neurodegeneration.

IMPORTANCE: Tau positron emission tomography (PET) can detect and stage Alzheimer disease pathology; however, current binary regulatory-approved visual interpretation focuses on advanced disease, limiting early detection and patient stratification. OBJECTIVE: To develop and validate a harmonized tau PET visual reading framework across radiotracers that stages tau burden into 5 classes: negative, low, moderate, high, and atypical. DESIGN, SETTING, AND PARTICIPANTS: This prospective multicenter cross-sectional study among a cohort with head-to-head tau PET acquisitions was conducted from March 2022 to August 2025 and data analyzed from February to June 2026, with blinded visual reads performed by clinicians. The study was conducted at 9 sites across North America and Europe. Of those individuals enrolled in the HEAD (Head-to-Head Harmonization of Tau Tracers in Alzheimer's Disease) study, a proportion of participants aged 50 to 90 years who completed multitracer tau PET (2-4 scans) and cognitive assessments were included, spanning cognitively unimpaired to dementia. EXPOSURES: Tau PET with 18F-MK-6240 (TAUKLARIFY, n = 681), 18F-flortaucipir (Tauvid, n = 644), 18F-RO948 (n = 150), and 18F-PI-2620 (n = 149); all participants underwent amyloid-β (Aβ) PET and magnetic resonance imaging. MAIN OUTCOMES AND MEASURES: The primary outcomes were interrater and intertracer agreement for 5-class tau classification; prevalence ratio (PR) vs the regulatory-approved 18F-flortaucipir framework; and associations with cognition, Aβ burden, and plasma phosphorylated tau 217 (p-tau217). RESULTS: Of 822 individuals enrolled in the HEAD study, 681 participants spanning cognitively unimpaired to dementia were included. Mean (SD) age was 69.3 (8.4) years, and 367 participants (53.9%) were female. The harmonized 5-class framework achieved excellent interrater agreement (κ = 0.77-0.85) and good to excellent intertracer agreement, highest between 18F-MK-6240 and 18F-flortaucipir (κ = 0.88). Compared with the regulatory-approved framework, the harmonized classification identified more tau-positive cognitively unimpaired participants (regulatory-approved binary: 26 of 364 participants; harmonized 18F-flortaucipir: 49 of 364 [PR = 1.89; 95% CI, 1.31-2.70; P = .001]; harmonized 18F-MK-6240: 71 of 364 [PR = 2.73; 95% CI, 1.95-3.83; P < .001]) and those with mild cognitive impairment (regulatory-approved binary: 100 of 223; harmonized 18F-flortaucipir: 123 of 223 [PR = 1.23; 95% CI, 1.12-1.35; P < .001]; harmonized 18F-MK-6240: 142 of 223 [PR = 1.42; 95% CI, 1.27-1.59; P < .001]), with convergence in dementia. Tau burden classes showed stepwise worsening of cognition, Aβ PET burden, and plasma p-tau217, with 1.4-fold, 3.2-fold, and 2.5-fold differences between the low and high tau classes, respectively. CONCLUSIONS AND RELEVANCE: In this prospective multitracer cross-sectional study, a 5-class harmonized tau PET visual reading framework showed high interrater and intertracer agreement and detected more early-stage tau pathology than the regulatory-approved binary approach, with tau burden classes tracking stepwise with cognition and biomarkers. These findings support a standardized, tracer-agnostic framework for diagnostic categorization, therapeutic window identification, and clinical trial stratification.

BACKGROUND: Timely detection of cognitive impairment due to Alzheimer's disease (AD) is increasingly important as disease modifying therapies become available. Current diagnostic pathways in primary health care (PHC) rely mainly on clinical assessment and brief cognitive screening tools, which lack sensitivity for early impairment. Blood-based biomarkers (BBMs) show promising diagnostic potential in research settings. Targeted screening of individuals with cognitive symptoms and dementia risk factors using sensitive cognitive tests and reliable biomarkers may improve early AD detection in PHC. The PREDEM primary care study evaluates a targeted screening strategy combining a multidomain digital cognitive test battery and BBMs in individuals having elevated risk of dementia. This pilot study assessed the feasibility of recruitment, digital cognitive testing, biomarker sampling, and data collection. METHODS: This descriptive pilot study was conducted at two PHC centres in Stockholm (April-October 2024). Eligible individuals were aged ≥ 65 years with hypertension or type 2 diabetes and self-reported memory complaints. Participants completed a screening question, followed by a demographic questionnaire, a multidomain digital cognitive test battery (Mindmore), MMSE, and blood sampling for four BBMs (GFAP, NF‑L, total tau, pTau217). Feasibility metrics included recruitment yield, test completion rates, participant characteristics, cognitive performance, and biomarker distributions. RESULTS: Of 548 identified individuals, 307 were invited and 40 were enrolled (13% participation). Among those enrolled, 37 completed all parts of the study. Digital home-based testing proved challenging: although 22 participants initially opted for home testing, only six completed it independently, and most required clinic-based support. Across cognitive tests, approximately one-third of participants scored below 1 standard deviation of normative performance, indicating detectable cognitive impairment. Non-native Swedish speakers (32%) showed significantly lower performance on verbally mediated tasks. Established cut-off values for pTau217 reflecting amyloid pathology showed pathological levels in 10-23% of participants depending on single or double cut-offs. CONCLUSIONS: This pilot study demonstrates the feasibility of the PREDEM primary care study design and data collection procedures. Targeted recruitment of older adults with cardiometabolic risk factors was able to identify a population relevant to the study aims. Key protocol refinements include removing the requirement for subjective memory complaints, conducting all cognitive testing at PHC, and optimization of cognitive test battery.

Toward development of an autologous, induced pluripotent stem cell (iPSC)-based cell therapy for Parkinson's disease (PD), we demonstrate successful, reproducible genomic and transcriptomic qualification of patient-derived dopaminergic neuron precursor cells (DANPCs) across multiple donors. Our analysis includes whole-genome sequencing data from fibroblasts, iPSCs, and DANPCs and the development of NeuriTest, an RNAseq-based bioinformatic analysis of DANPCs designed to predict cell quality based on empirical animal data. Autologous cell therapies are immune matched to the patient, potentially augmenting durability of benefit compared to allogeneic cells while negating the need for immunosuppression and accompanying side effects. Patient-specific iPSCs are an autologous cell source that can be differentiated to dopaminergic neurons, the cell type lost in PD. We report here our preclinical manufacturing strategy and results demonstrating efficacy in a PD rodent model and safety in a 9-month GLP toxicology study.

BACKGROUND: Alpha-synuclein accumulation contributes to Parkinson's disease (PD), and lowering α-synuclein expression is a candidate disease-modifying approach. The brain-wide pharmacodynamic profile of human-targeting antisense oligonucleotides (ASOs) remains incompletely defined in humanized models that preserve the regulatory architecture of the human SNCA locus. METHODS: BAC-hSNCA rats were treated at an early stage, when human α-synuclein is elevated, but striatal dopamine content remains largely preserved. A single intracerebroventricular dose of an ASO targeting human SNCA 3'UTR (SNCA ASO3.0) was administered, and outcomes were assessed 45 days later by isoform-specific RT-qPCR, immunoblotting of soluble and detergent-insoluble α-synuclein species, bulk RNA-seq across selected regions, multiplex cytokine profiling, phospho-kinase assays, neurotransmitter quantification, and behavioral testing. RESULTS: SNCA ASO3.0 reduced human SNCA mRNA and multiple α-synuclein protein species in a regionally graded manner, with the strongest effects in the olfactory bulb and striatum. Bulk RNA-seq identified anatomically distinct transcriptional responses, including synaptic and metabolic pathway shifts in the olfactory bulbs and immune-related enrichment within glial signatures in the striatum and midbrain, without detectable changes in protein levels of glial fibrillary acidic protein or ionized calcium-binding adapter molecule 1. Cytokine profiling showed selective modulation of a subset of analytes, and phospho-kinase assays indicated reduced AKT1S1/PRAS40 phosphorylation. Striatal dopamine levels were unchanged, whereas the striatal tyrosine hydroxylase level was increased. SNCA ASO3.0 increased locomotor activity and improved exploratory behavior and selected measures of olfactory discrimination. CONCLUSIONS: Partial suppression of human α-synuclein in a humanized rat model yields region-specific molecular adaptations accompanied by early behavioral changes at a stage preceding overt dopamine depletion. These data provide a framework for evaluating region-resolved consequences of α-synuclein-lowering interventions in synucleinopathies.

The LRRK2 G2019S mutation, a Parkinson's disease-linked variant, has been associated with depression-like phenotypes, but mechanisms remain unclear. We chart age-dependent behavioral changes and astrocyte reactivity in G2019S mice and define a pathway connecting LRRK2 to TFEB/GDNF signaling, cellular energetics, and inflammation. From ∼25 weeks, G2019S carriers show robust depressive-like behaviors with reduced hippocampal GDNF, exaggerated astrocytic inflammation, diminished ATP, and increased neuronal apoptosis; pharmacologic LRRK2 kinase inhibition reverses these abnormalities. Single-cell transcriptomics identifies 507 differentially expressed genes enriched for adenylyl cyclase/cAMP signaling, oxidative phosphorylation/ATP metabolism, calcium/ion homeostasis, and cytokine pathways, indicating disrupted cAMP-ATP coupling and perturbed neuroimmune signaling. Mechanistically, LRRK2 G2019S binds and suppresses TFEB while elevating Ser211 phosphorylation, lowering astrocytic GDNF, amplifying inflammation, and reducing ATP. Critically, TFEB overexpression restores GDNF, elevates astrocytic ATP, attenuates inflammatory mediators, and improves depressive-like behaviors. Enhancing effector nodes is therapeutic: bilateral hippocampal forskolin rapidly (within 6 h) raises cAMP and reverses multiple behavioral measures, and ATP supplementation (systemic or hippocampal) yields comparable benefits. These findings position a LRRK2→TFEB/GDNF→cAMP/ATP axis as a driver of astrocytic inflammation and energetic imbalance underlying depression in G2019S mice and highlight convergent therapeutic strategies targeting LRRK2, TFEB, and ATP/cAMP.

In-depth characterization of pathological protein aggregate with its subcomponents is pivotal for early diagnosis and staging of neurodegenerative disorders. Here, we introduce the Protein Aggregate NanoDynamics Analyzer (PANDA), a plasmonic nanotechnology-powered system that translates the supramolecular size distribution of protein aggregates into distinct scattering signatures via sterically constrained immunogold clustering. In this work, PANDA enabled the profiling of α-synuclein (α-syn) aggregates with diverse supramolecular architectures from human serum samples and experimentally revealed biological stage-dependent distribution patterns in Parkinson's disease (PD). Aggregate size profiles from PANDA readouts showed strong correlation with clinical scores (rmax = 0.6) and dopaminergic PET imaging (rmax = 0.7). Notably, it also confirmed the pathophysiological transition of "oligomer-to-fibril" in the course of PD progression. To leverage this transition, we incorporated a deep neural network (DNN) to classify PD stages. The network achieved high accuracy and enabled an objective reference to evaluate PD progression. PANDA system thus offers a noninvasive, artificial intelligence (AI)-augmented framework for molecular diagnosis and stratification of neurodegenerative diseases such as PD.

BACKGROUND: Parkinson's disease (PD) is a clinically and biologically heterogeneous neurodegenerative disorder, driven by multiple mechanisms among which are lysosomal and mitochondrial dysfunction. Here, we explored blood-based lysosomal and mitochondrial profiles in relation to PD diagnosis and severity. METHODS: Plasma samples (n = 504) were analyzed using SomaScan, targeting 394 lysosomal and 502 mitochondrial proteins in a discovery (ProPARK: 145 PD, 73 controls) and a validation cohort (Alzheimer's Disease Research Center [ADRC]: 104 PD, 182 controls). Linear and LASSO regression were included to compare groups and select PD-predictive proteins. Associations with Unified Parkinson's Disease Rating Scale (UPDRS), Hoehn and Yahr, Montreal Cognitive Assessment (MoCA), levodopa equivalent daily dose (LEDD) and disease duration were assessed and age-, sex-, and false discovery rate (FDR)-adjusted. RESULTS: In ProPARK, 29 dysregulated lysosomal and 46 mitochondrial proteins were identified in PD; upregulated lysosomal sulfatase-modifying factor 1 (SUMF1) (FC = 1.41) and arrestin domain containing 3 (ARRDC3) (1.36) survived correction. LASSO identified eight lysosomal proteins (area under the curve [AUC] train: 0.83, test: 0.75) and two mitochondrial proteins (AUC train: 0.66, test: 0.65). Lysosomal score associated with disease duration, LEDD, and UPDRS-IV, whereas mitochondrial score did not. SUMF1 associated with disease duration, LEDD, and UPDRS-IV; ARRDC3 only with disease duration. In ADRC, upregulation of SUMF1 and ARRDC3 in PD (FC = 1.72 and 1.36), their association with LEDD, discriminatory AUCs of lysosomal and mitochondrial scores (0.80 and 0.58), and the association between lysosomal score and LEDD replicated. CONCLUSIONS: Lysosomal expression scores discriminated PD from controls and associated with disease severity. SUMF1 and ARRDC3 emerged as blood-based biomarkers for PD. These findings support the use of pathway-specific biomarkers for improving PD diagnostics and monitoring disease progression. © 2026 The Author(s). GBA1 variants represent some of the most important genetic risk factors for Parkinson's disease (PD), present in a substantial proportion of patients across populations. This review provides a comprehensive overview of the field, tracing the historical path from the first description of Gaucher disease (GD) in 1882 to the recognition of GBA1 as a key contributor to PD risk, and highlighting major scientific and translational milestones along the way. We summarize current understanding of the biological mechanisms linking GBA1 to PD, including both loss-of-function and gain-of-function hypotheses, and discuss how reduced glucocerebrosidase (GCase) activity may lead to α-synuclein accumulation and neurodegeneration. Remaining gaps in knowledge, especially regarding downstream lipidomic effects and cell-type-specific mechanisms, are emphasized as key priorities for future research. This review also examines clinical and translational advances, including the development of GCase-targeted therapies such as small molecules, chaperones, substrate reduction therapies, and gene-based approaches. We discuss outcomes from recent and ongoing clinical trials, highlighting both promising strategies and notable failures that inform our evolving understanding of disease biology. In addition, we address the heterogeneity of PD and the importance of stratifying patients based on genetic background. Finally, we outline critical challenges and future directions, including the need for precision medicine approaches based on variant-specific mechanisms, improved biomarkers to define true GBA1-associated PD, and optimized clinical trial design. We propose that coordinated, open-science efforts and large-scale collaborative frameworks will be essential to accelerate discovery and translate mechanistic insights into effective disease-modifying therapies.

OBJECTIVE: Dementia with Lewy Bodies (DLB) is the second most common neurodegenerative dementia after Alzheimer's disease (AD), with significant pathological overlap between the two conditions. However, the impact of Alzheimer's co-pathology on basal forebrain (BF) atrophy and its relationship with neurodegeneration and cognitive decline in patients with DLB remains underexplored. METHODS: Forty patients with DLB and 14 healthy controls (HCs) were included from the Centre for Neurodegenerative Diseases, University of Bari. Participants underwent neuropsychological evaluations, 3T magnetic resonance imaging (MRI) scans, and Alzheimer's biomarkers assessment. Patients with DLB were grouped by AD co-pathology using in vivo biomarkers. BF and hippocampal volumes and mean cortical thickness values were analyzed using analysis of covariance (ANCOVA), adjusted for age, sex, and total intracranial volume. Mediation analyses were conducted to examine the indirect effects of Alzheimer's pathology and BF atrophy on neurodegeneration (hippocampal volume and cortical thickness) and cognitive function (Mini-Mental State Examination [MMSE]). RESULTS: Patients with DLB exhibited significant BF atrophy compared with HCs (p < 0.001). AD co-pathology was associated with greater BF atrophy (p = 0.046) and reduced mean cortical thickness (p = 0.025). Significant correlations were found between BF volume and the pTAU181/Aβ1-42 ratio (p = 0.009) and between BF volume and MMSE scores (p = 0.016). Mediation analysis revealed that BF atrophy mediated the relationship between the pTau/Aβ42 ratio and neurodegeneration, and that the BF has a direct association with cognitive impairment. INTERPRETATION: AD co-pathology in DLB exacerbates BF atrophy, correlating with cognitive decline. These results emphasize the need to consider AD co-pathology in evaluating neurodegeneration in DLB, suggesting future studies should explore targeted interventions to address this pathological overlap. The semi-quantitative assessment of brain [18F]FDG PET provides a more objective interpretation and improved accuracy in differentiating across neurodegenerative diseases. However, the correct identification of anatomical regions of interest without a structural MRI is challenging. Thus, this study aims to develop a deep-learning-based (DL-based) model for the automatic segmentation of 52 anatomical regions in brain [18F]FDG PET images and validate it across different metabolic profiles. 1628 brain [18F]FDG PET images of 1099 subjects were included in the internal dataset, comprising cognitively normal subjects (n=537), and patients with mild cognitive impairment (n=538), subjective memory concerns (n=70), Alzheimer's disease (n=330), frontotemporal lobar degeneration (n=95) and Lewy body dementia (n=58). Train-test split yielded 1109/519 images (631/468 subjects) for training and internal testing of a DL-based model, respectively. An additional dataset of 108 [18F]FDG PET images was included for external validation. Ground-truth segmentation was performed on the paired T1-weighted MRI image for each [18F]FDG PET image, for a total of 52 anatomical regions of interest. Atlas-based segmentation was used as a benchmark. The Dice similarity coefficient (DSC) was used to assess segmentation performance. Agreement in mean pons-based standardised uptake value ratio (SUVRmean) quantification was assessed through the intraclass correlation coefficient (ICC) and relative deviation in absolute value. Per-region mean DSC for the DL-based segmentations ranged from 0.729 to 0.923 in the internal test set. Global mean DSC was 0.84±0.06. SUVRmean quantification of the different regions using the DL-based segmentation masks showed strong agreement with that obtained using the ground-truth segmentation masks, with an average ICC of 0.96±0.02. The per-region average of relative SUVRmean deviation did not exceed 5%. DL-based segmentation significantly outperformed atlas-based segmentation (p<0.05). Similar segmentation performance was obtained in the external validation dataset. DL-based anatomical segmentation of brain [18F]FDG PET proved to be robust across a wide spectrum of neurodegenerative diseases. Semi-quantitative assessment was comparable with that obtained with MRI-based segmentation. DL-based segmentation, therefore, proved to be a reliable alternative when MRI isn't available, and is a better option than the commonly used atlas-based approach.
